Probably belongs in 'diving related lessons'...not really a 'technical' dive.

What makes it even more tragic is that should not have happened.

Wow. Shocking and sad. Thanks for putting up the article. I read about it a while back, and I wondered exactly what had happened. Something like this never would have been my guess.
"The most important thing is not to stop questioning." Albert Einstein

"For the diligent diver, closed circuit rebreathers are actually safer than open circuit scuba." Tom Mount

I'm sure they have to use some pretty specialized gear to do this kind of diving, even if it was just a checkout/experience dive. However, that does not remove the divers themselves for the responsibility of making sure their air hoses are attached to buoyancy devices, weighting themselves properly, etc. I just don't get that at all. If you were underweighted with 30 lbs, you wouldn't DOUBLE your weight on the next dive, would you? Maybe just add a few lbs but not 30 lbs!

As for the gear not having been expected in so long, that is insanity and the CG/boat and dive staff is certainly responsible for that. I am not prepared to judge how impaired the dive watch team was (the guys feeding the air hose) based on what was said, but it certainly appears there was some sloppiness there.
Back after a long absence - whew, too busy at work!

exactly what I thought - it's been moved.
It's Winter time - you know you're a diver when you're scraping ice off your windshield INSIDE your vehicle...!

Note not only the excessive weight but that there was no BCD LP hose leaving only the suit for bouancy.
With the exception of the FFM and being teathered the gear itself is not outside of the recreational diving relm.
It was a planned shallow dive yet enough line was fed out to reach some depth...a good dive tender take skill. It isn't a matter of just taking someone and explaining the pulls then letting 'em loose
