Many divers have problems planning thier dives, plan your dive and dive your plan, I know I did-so to help myself out I put together a dive planning worksheet. Now this is a project of mine and it has been under constant revision. You are welcome to use it and also to make suggestions. There is space for you to record your Emergency Action Plan, sketch your dive route, develop your dive brief so on and so forth. Let me kow what you think

I like the concept a lot. Looks like something I saw in the Red Sea, but it was large on a white board so the DM's could fill it in for shore dives.
The EAP area should be a little more prominent. Red or black/yellow border or something like that so it stands out. You may have a non-diver as designated emergency person on the shore... A number for Emergency Services (Typically 911) should be on there in addition to DAN as that would be your priority. I like the direction to dive site part. Perfect for ambulance or fire response.
Things we do for our shore dives are designate an emergency vehicle and key location. Emergency cell phone location. Coast guard / Rescue Coordination Center phone numbers, etc.
There is no place for an emergency recall signal.
Why the graphics for multiple level dives, PG and SI on both pages? That info would not be known until after the dive, yes? Not being critical, just curious...

and THANK YOU for sharing! Many people tend to not share such creations! Excellent. I will rummage through and find an emergency assistance plan form one of my fabulous new Divemaster Candidates came up with for shared use!

I ust it to plan my dive profile for the day and get a rough idea of how long surface intervals will be so on and so forth and work a time speradsheet. I prepare my dive classes using this method then I adjust the data from the actual dive and record it in my log book, for exapmle here is a plan for an AOW class. As for the EAP there is room for all the info required, I have a sheet like this with EAP for each site I dive. I insert uw maps for standard locations. Since I am the only one using this it is easy for me to work, but I will incorporate some of your ideas. As you can see, being single I have WAY too much time on my hands

I have made some changes to my planner as suggested by dive girl, I leave alot of the EAP blank so you have place to record the information for each dive site you go to. I loeft 911 off cause well gee we all know 911 dont we?
And you should determine your dive profile before you dive should you not? and then record what you actually do in your log book. At least thats what I do. Give me a rough idea of what my day will be like

I left the EAP block vauge on purpose so each diver could include details based on thier particular situation. This is base document and is intended to be flexible
