Just a few more things that have been reported:
Owner of the cylinder stated that the cylinder had a current hydro and visual (Dont know who performed either). Also, the cylidner was being used to carry oxygen for decompression gas (and marked as such). It was not a medical cylinder, but a standard scuba cylinder. Cylinder had about 1200 psi of O2 immediately prior to the incident. There is a pic of the cylinder after the incident on TDS (if anyone wants to wade thru the crap to find it).

Since we now know it was O2 the next questions I would ask are what was the pressure in the tank just prior to the accident, If a booster pump was being used what was the supply pressure and how fast were they cycling the pump? We may never know but I am interested.

BTW pardon my stupidity but what is TDS?

Cylinder reportedly had 1200 psi of O2 in it. Cylinder was connected to the whip, but not being filled, reportedly. Its tough to say if any of that is factual though, because we cant really be sure where that information originally came from...

I guess we wont really know exactly what was going on since the primary witness is gone. Its too bad things like this happen but I will be using it as an example in the next VIP, Blending or O2 technition course I teach. Photo's hammer the point in a little more than warnings.

i,m new to your site and i got to say after reading all this about the tank exploding , i'm shocked that no one said anything about the woman that loss her life i mean think about it some ones husband is sitting at home and his wife is never coming home and what if she had kids thell never get to see ther mom again !!!!! i know what its like to loose some one close to you it really sucks and my loss was about three years ago it still hunts me cuz i'll find my self saying that she'll be home any time then realty sets in and i got to live it all over again

Actually, the first reply stated it was a tragedy. After that, the posts have been about the circumstances, hopefully to understand the causes and avoid such situations in the future.

You are correct. If this were merely about property damage, it wouldn't be nearly as serious. Death and injuries are terrible and we hope to avoid such accidents in our sport.
