Hello please check out the new form we have created to help us book airfares for you more easily and ensure 100% accuracy! Let us know what you think and if we need to make any improvements to it. Your input helped us create the new form AND the new format...so help us improve the new format as well!

p.s. Please note that we will be making ALL our forms fillable pdf's so you can fill them out online and then fax or email them to us. But we are starting small to be sure we do it right!

Not sure if this was directed as me, the only thing I saw was that I was unsure what date format to use for both my birthday and my passport expiration date. The dates in my passport are in the format DD-MMM-YYYY, but you can't use that for the expiration date. I used MM/DD/YYYY.

Otherwise it worked fine!

I think we are going to add an email button so you can just fill it out and email it to us but we are working on that!

The challenge with that is when you ask for a signature, as you do on the waiver. So that pretty much forces you to print it out, sign it, and then scan it back in to send or fax.

Yes that is one of the things we are looking at as well as how to do it!

I am filling this out for the Dominica trips charter flight and noticed the passport # shows with commas in it and there's no place that specifically asks for the SD screen name.

I don't know that you would need the screen name but if so since there's a spot for name and full legal name on passport the name field could be used for screen name too.
