Cecile I apologize for overlooking the actual forwarding of your EV Boat Form until today based on feedback from EV Kamala posted in the Trip Private Message (PM) thread. I can confirm your completed form was indeed forwarded to EV mere minutes ago. Again my apologies for any concerns this has caused you.

• Margaret – The photos of her forms were too blurry to read

Updates needed for Application / Liability release forms:

• Tami  – Please ask her to hand-sign her liability release page next to her digital signature and resend prior to departure. 

Flights:

• John H – He provided the departing flight of AA1221, which is a Chicago (ORD) > Providenciales, TCI (PLS).  Please ask him to provide his flight departing from Providenciales (PLS) on January 18.  An amended form with updated flight information was sent 27 Dec
• Mark H – The departing flight he provided (DL1654) is an Atlanta (ATL) > West Palm Beach (PBI).  Please ask him to provide his flight departing from Providenciales (PLS) on January 18.
