If taking a class with our very own SD instructor Bruce aka ScubaShafer, here are the documents you will need for his classes.

When you fill these in, make sure you keep the originals and bring them with you. It is a good idea to make a copy for your own records.

NOTE: The EAN Liability Form is only required for the NITROX class and not required for any other class type.

If you have any questions, contact Bruce directly as you cannot post in this forum.
