Good catch! We need to use the RAB form where you tell us what your expectations are for the program etc. Can you do a help ticket in our system requesting it. I will be an optional form and we'll get it built and loaded in the system.  There is now a link to the help desk at the top of the forums!

Hi- I am responding to the email for updated information.  Payments made- 3/20 $500 deposit, 4/20 $500 progress, 5/20 $395 final payment on d/o diver package.  My documents were verified received (faxed) by Kamala upon my inquiry.  I am now nitrox certified, so put me on list for $130 basic dive pkg.  Also, put me on list for wall dive for $40.  I will pay the total of $170 remaining before 6/20 deadline.  I will be participating in costume contest.  I am already on list for cochran dive computer for free/DAN research.
