[Cajun Diver]

I have figured out how to set up a form for a trip and have uploaded the form that was in the PM and set up a section for Dominica Ft. Young forms.

So for this trip the form can be handled on their My Trips page for download and upload.

 

Edit - note I made the form name more linux friendly so replaced spaces either with an underscore or just removed it - FY_versionDiveWaiver_2020_updated_1.pdf.

 

I'll write up some instructions for how to do that.

Yes - I changed the "vendor medical" to yes, the form is there and they can download / upload from the My Trips section for the trip.
