I will have a short form in the private forum for you to download as well as in the forms forum. It is an insurance summary form. The liveaboard as well as SD takes its obligation to you seriously that we will be there to help you in the event of an emergency. This is even more important in our situation as we are traveling alone and only have each other to assist us in the event of an accident or illness.
Therefore we ask that you fill out the form provided which will ask you the following questions. (PLEASE DO NOT POST THIS INFO IN ANY OF THE THREADS NOR EMAIL THE INFO WITHOUT BEING IN THE FORM OR PM IT TO US. PLEASE fill out the form and send that to us via email or fax. AND bring a hard copy with you on the trip.)
1. Name and Full emergency contact info
2. Name of health insurance provider
3. Policy number and member number (A photocopy of your insurance card is also very helpful)
4. Number to call in case of an emergency
5. Process if any to follow to quickly contact your insurance company in the case of an emergency
6. If your policy is a 'secondary policy' meaning that other insurance steps in first and this one only covers what is not covered OR even if it is your only insurance, you will need credit access to pay for any treatments first and then you'll be reimbursed by your insurance later.
7. If your policy is a 'primary policy' then it means your policy will pay the providers directly and you are not involved. We do however need to know the process for doing this effectively.
8. Please prepare and bring with you in a SEALED envelope with your name on the outside a list of any meds you are on, anything you are allergic too, any health conditions a DOCTOR should know if you could not speak for yourself. It is imperative that you do this as I will be your only link to family, doctors, insurance companies etc. PLEASE MAKE IT EASY FOR ME TO HELP YOU!!!
TIME IS OF THE ESSENCE IN AN EMERGENCY!!! The time you spend making it easy for us to help you is the time it could take to save your life.
NOTE: PLEASE DO NOT SEND THIS INFO TO US IN ANY OTHER FORMAT OTHER THAN THE FORM WE ARE PROVIDING. These will be printed off and taken with us in the event they are needed. PLEASE carry it with you as well. And PLEASE bring your medical envelopes! (You might want to include a copy of your passport and c-card along with a copy of your insurance card(s) in this envelope too just in case!)

The following people have not responded to the posts on the site, nor emails sent personally by Tammy aka Landlocked Dive Nut. It is IMPERATIVE that I have this information for the boat and of course for our own use. In our seven years I've had 2 divers get bent on trips...I've escorted one diver back from overseas to the US and I've brow beat a chamber operator to send a diver back to the US BEFORE a hurricane was due to hit the country. In ALL cases time was lost not having critical information needed to start the 'chain of care' command. The medical envelope and this form are the cumulative results of these experiences AND the boat's need to reach YOUR medical provider quickly and effectively.
The time you spend NOW could be the time that saves your life. It is IMPERATIVE that we get this information from you. Please help us to help you should the unfortunate time arise we need it. and
p.s. This form will be used on all SD trips so once you do it...you're ready for the next time!
